RELEASE FORMS

BOTH a medical and a liability form should be completed for anyone who is participating in a day or overnight retreat involving recreation or swimming.

Group Leader: Download and print the 2 forms PLUS the Recreation Info and provide a copy to each participant in your group. Put completed forms in alphabetical order and mail (do not fax) to Deer Run Retreat no later than 14 days before your retreat date.
